The Case In Point Platinum Awards recognize the most successful and innovative case management programs working to improve health care across the care continuum. From the publisher of Case In Point, Case In Point Weekly, the Case Management Resource Guide, this first annual awards competition will set the standard for programs that deliver sustained success across a variety of disciplines and settings in the overarching continuum of care coordination.

A range of 25 categories, from best emergency department program to best independent case management program to dozens in between, provides recognition to the exceptional work and esteemed value delivered across the folds of case management.

We seek to honor those programs that best educate and empower patients, improve adherence and wellness, manage quality care, and contain health care costs. Be sure to submit your entry – and get recognized for all you do to improve wellness in America.

The Case In Point Platinum Awards will increase recognition of case management in the eyes of legislators, the health care team at large and the public by recognizing the best of the best.

 

Who Should Enter

This awards program is open to any hospital, insurance company, or health care organization that has sustained, launched, built or initiated programs that have made an impact in health care and the field of case management.

 

Eligibility Requirements

The awards program is open to any case management program in the United States and its territories that fits within the categories below. You need not belong to a professional organization to apply. You may apply in more than one category. Once you choose the category that best matches your program, complete the entry form to ensure your organization is entered for consideration. Self-nominations are accepted.

 

Categories

The best case management programs will be selected from the following categories:

  • Behavioral Health Case Management – Recognizing the best program for coordination and management of care for patients with various behavioral health conditions.
  • Case Management Provider Program – Entries for best provider-based case management program is one that strives to ensure resources are coordinated and utilized effectively and efficiently.
  • Disability Case Management – Seeking the best disability case management programs for working with patients who are on short- or long-term disability.
  • Discharge Planning Program – An award for inpatient facilities that ensure patients transition safely from one setting to another. 
  • Disease Management/Population Health – For the program that manages patients with chronic medical conditions in order to decrease exacerbations and improve quality of life.
  • Emergency Department – Saluting the emergency department case management program that ensures patients have access to the most appropriate setting to match their health care needs.
  • Hospital Case Management – This category seeks the best hospital case management program that works collaboratively with patients and the health care team to ensure patients receive safe, quality, evidence-based care in the least restrictive setting.
  • Independent Case Management – Recognition of entrepreneurial case managers who contract with employers, managed care organizations and consumers to advocate and manage patients with catastrophic and chronic medical conditions.
  • Integrated Case Management – The best integrated case management program that acts as the payer as well as the provider to provide care for patients across the continuum. 
  • Long-Term Care – The top program that works with patients in long-term care settings such as skilled care facilities, continuous care retirement centers, and nursing homes.
  • Managed Care – Seeking programs where the care provided is evidence-based, safe and delivered in the least restrictive setting.
  • Medicaid Case Management – Recognizing case management programs that focus on the Medicaid population.
  • Medicare Advantage Program – Recognizing case management programs that focus on the Medicare population.
  • Military Case Management – Providing the best in care coordination for active, retired and dependents involved in the U.S. military.
  • Nurse Call CenterHonoring the best call center that provides patients an effective and efficient resource to gain information and guidance regarding health and heath care issues.
  • Occupational Health Care Management – Honoring the top professionals who keep workers well through prevention programs and medical management programs.
  • Pediatric Case Management – Working with pediatric patients and their families to ensure care coordination throughout the care continuum.
  • Rehabilitation Case Management – Top in the field at assisting patients who are undergoing rehabilitation to return to their maximum function.
  • Safety and Quality Management Program – Recognizing the best at ensuring care to patients with safe, effective, evidence-based care.
  • Social Work – An outstanding social work case management program that coordinates care as well as resource management throughout the care continuum.
  • Utilization Management Program – Excellence at ensuring health care resources meet evidence-based requirements and the specific needs of the patient.
  • Wellness/Prevention – The best program for ensuring members understand their specific risks and the prevention strategies to improve health and wellness.
  • Women/Children Case Management Program – Honoring women and children case management programs that address the needs and resources of this targeted population.
  • Workers’ Compensation Case Management – For the program at the top of the list that strives to return the injured worker to gainful employment through medical management programs.
  • Overall Case Management ProgramThis prestigious honor recognizes the leading case management program across the continuum, regardless of setting or discipline. This is an all-around category and any area of case management or a combination of practice areas may apply. If you have made an impact by increasing access, improving quality or reducing cost in a global manner, this is the category to which you should apply. All entrants can be considered for the Overall Case Management Program award, but it is necessary that the submitter designate it as a primary or secondary category.

 

How to Enter

Download the entry form. Professionals can self-nominate in one or more of the listed categories. Nominations need to be submitted with the following information and postmarked by December 11, 2009:

  • The department head’s name and position.
  • Name of organization.
  • The submitter’s (contact person) name, daytime phone number and email address.
  • A brief description of your program and a synopsis of the most compelling reasons why your program deserves to win in the category of your choice (no more than 750 words). Please provide examples of how your program demonstrated stellar achievements over the past year. Please include, where possible, measurable data and supporting materials, such as outcomes, statistical evidence, press notices and other documents that evince your "best-of" status.

You may consider dilating on the following as you prepare your synopsis:

  • Within your program, are you communicating with the patient, family and others to gain a comprehensive assessment?
  • Does your program include a plan of care that includes short- and long-term goals and incorporates the patient/family needs?
  • Does your facility transition the patient as needed and provide information that enables the patient and/or the next case manager to understand the plan of care and the unique opportunities or challenges that have been discovered?
  • Are you advocating for the patient and members of the health care team to ensure the plan of care is meeting the needs of the patient and that progress is being made? If barriers are identified, is the plan of care re-evaluated?
  • Are care coordination activities and short- and long-term goals documented and are you actively monitoring the plan of care as the needs of the patient change? Are cost-savings and outcomes reported?

Send three (3) copies of your entry form, five (5) copies of your synopsis, one (1) set of supporting materials and the entry fee.

Deadlines/Entry Fees

Early Deadline: December 11, 2009 (postmarked)
Late Deadline: December 18, 2009 (postmarked)

The price of each primary entry is $290. The price of additional entries is $145. A late entry surcharge of $150 will be applied per entry submitted after December 11. Payment in full must accompany the entry. Entry fees are not refundable.

Award winners and honorable mentions will be notified by January 31, 2010, and honored during the awards ceremony on April 20, 2010, at the National Press Club in Washington, D.C.

 

FAQs

How are the entries judged?
Entries are judged by a panel of experts in the health care field from the clinical side and the business side of health care as well as those who are familiar with the roles and responsibilities of today’s case manager. Also serving as judges are the staff of Case in Point Magazine and Case in Point Weekly. We evaluate your entry based on the proven track record and success of your program.

Who is eligible to win an award?
All case management departments and programs throughout the United States and its territories are eligible to enter and win an applicable award.

Where and when will the awards ceremony take place?
The 25 winners and additional honorable mentions will receive their Case In Point Platinum awards and will be honored at a luncheon on April 20, 2010 at the National Press Club in Washington, D.C.

How can my program increase its chances of winning an award?
Provide the requested information with a thorough essay on why you deserve to win and include supporting materials like outcomes data and press notices.

When will we be notified about the status of our entry?
Your department or contact person will be notified if you are named a finalist for your category. Finalists will be notified by January 31, 2010.

Are the awards affiliated with any professional organizations?
The Case In Point Platinum Awards is an independent competition and your submission will not be shared with any outside parties.

What if I want to submit to more than one category?
Please do. We encourage you to submit to all categories to which your program or department applies.

What are the entry fees?
The price of each primary entry is $290. The price of additional entries is $145. A late entry surcharge of $150 will be applied per entry submitted after December 11 (and no later than December 18).
