How to Enter and What To Send
Eligibility Requirements
The healthcare team consists of diverse professionals, each with an important role. The nomination categories highlight the key areas of care coordination and transitions of care.
How to Nominate
Professionals of the care coordination team can nominate a peer, mentor, supervisor or themselves in one or more of the listed categories. Nominations need to be submitted with the following information and postmarked by June 10, 2011:
In 1,000 words or less, please provide objective data that conveys the nominee's abilities and/or achievements in the categories listed:
- Overall achievements will be considered, as well as single, one-time innovative programs that demonstrate outstanding results.
- Specific programs or efforts you have spearheaded to improve health and healthcare of consumers as a member of the care coordination team in your practice/organization.
- Leadership qualities (ability to manage and mentor, internally, intra-departmentally and externally).
- Other relevant qualifications such as creativity, critical thinking and successful financial management in your organization.

Five (5) copies of your 1,000 word synopsis

Resume/Curriculum vitae of the nominee (optional and cannot replace the 1,000 word synopsis)
Deadlines/Entry Fees:
Early Deadline: June 10, 2011 (postmarked)
Late Deadline: June 17, 2011 (postmarked)
The price of each primary entry is $290. The price of additional entries is $145. A late entry surcharge of $150 will be applied per entry submitted between June 10 and June 17, 2011. Payment in full must accompany the entry. Entry fees are not refundable.
Finalists will be notified by July 17, 2011, and honored during the awards ceremony on October 20, 2011 at the National Press Club in Washington, D.C.
Judging Process
A panel of judges will review each submission from the documentations submitted. Overall achievements will be considered, as well as single, one-time innovative programs that demonstrate outstanding results.
Who Should Enter
The Dorland Health People Awards competition is open to each member of the healthcare team responsible for managing health and healthcare. The People Awards seek to honor professionals who stand out for their diligence, their dedication, their leadership, their outcomes production, their overall provision of patient care, or any or all of the above.
Recognition:
Dorland Health People Awards winners will receive:
- Engraved, personalized award
- Recognition at the national awards event on October 20, 2011 in Washington, D.C.
- People Award Winner logo to use for marketing purposes
- Coverage in the October issue of Case In Point magazine, DorlandHealth.com, and CIP Weekly
- Promotion via press releases and other marketing initiatives
