 HHS report: Hospitals fail to report adverse events
Hospitals are not following laws requiring them to report adverse events in which patients were harmed, making it difficult to identify and correct problem areas, according to a report by the HHS Office of Inspector General. The report said the problem is that staff do not know what the regulations are and often identify cases as expected side effects rather than adverse events.
