Drug shortages hit record levels last year and have led to patient injuries from mistakes and at least 15 deaths in the U.S. since mid-2011, the Institute for Safe Medication Practices reported. Shortages came from drug manufacturers that stopped production or had production problems, forcing hospital pharmacies to scramble to ensure uninterrupted supplies of critical medications for cancer, heart attack and other seriously illnesses.
