Program Objectives

  • Provide a course of study to assist professionals and other stakeholders in understanding the dynamic role of case management in today’s healthcare system.
  • Address common competencies, roles and settings relevant to all involved in the practice of case management across the care continuum.
  • Discuss the challenges that impact the practice and strategies to improve the practice.
  • Explain the competencies needed to be successful as a case manager.
  • Learn the core components of the practice and how they transfer from setting to setting.



Course Outline:

Session 1: The Current Healthcare System and Applications to Case Management
In this session, we will look at the trends and issues impacting the current healthcare environment and share insights into why the practice of case management is positioned to improve access and quality while containing escalating healthcare costs.

Advanced Learning: Stepping Up To Certification provides a resource for professionals to better understand the certification process for medical case management, insight into the various certifications available and a guide to help professionals prepare for case management certification.

Session 2: History of Case Management
We take a deep dive into the history and background that has shaped the current practice of case management and look at the philosophy and goals case managers in all settings strive to meet. We will also look at the current practice settings that span the continuum of care.
Session 3:

Core Competencies and Responsibilities Across the Continuum
Case managers may differ according to practice settings, but all have a common role and function that has been documented through certification bodies as well as the Case Management Standards of Practice. In Session Three, we will explore these areas and provide case examples to illustrate each role and function so that learners can better understand the practice.

Advanced Learning: Focusing The Lens on Care Management Outcomes provides case managers, social workers, rehabilitation professionals and other members of the care continuum team insight and actionable data on topics critical to the successful outcomes of patients. Case studies, recommendations and best practices from top healthcare professionals contribute to the knowledge you can gain regardless of your setting.

Session 4: Rehabilitation, Workers’ Compensation and Disability Management
An important role of the case manager is to ensure patients receive appropriate care to allow them to reach their full potential. Doing so enables people to work, support their families and maintain their dignity. In this session we will look at processes case managers can use to ensure timely return to work and learn about the various professionals who are part of the return to work process.
Session 5:

Data Management
Session Five will review the important role technology is playing in the area of care coordination and how it is changing the delivery of care. We will also look at how the documentation systems and evidence-based guidelines are decreasing variation of care and improving quality and efficiency within organizations and the system as a whole.

Advanced Learning: Take a few minutes to view Going Beyond the EHR: Supporting Care Coordination, a webinar that will share insights into why EHR adoption is a necessary component of physician practices and what enhancements are needed that will allow for proactive care coordination of patient populations around quality and outcomes.

Session 6:

Resource Management
Resource management is a key role for all case managers regardless of setting. In this session, we will review the various resources and the rules that case managers need to understand in order to ensure patients have access to the resources they need to meet their individual needs.

Advanced Learning: Motivational Interviewing: An Emerging Trend in Medical Management will explain how motivational interviewing is being used in a variety of healthcare settings to engage consumers and help them adhere to the plan of care and utilize healthcare resources more appropriately.

Session 7: Utilization Management
Efficient utilization of resource management is key to effective case management. Session Seven looks at the history of utilization management, the various rules, and how case managers can work with the team to ensure patients get the resources that are medically necessary to meet their needs.
Session 8: Population Health and Disease Management
Understanding population health and addressing disease management is critical to controlling escalating healthcare costs. This session will review principles and best practices that case managers can incorporate into practice regardless of setting.
Session 9:

Healthcare Legislation
Healthcare legislation provides a tool for case managers to ensure access to services and protects consumers who have medical, behavioral and social challenges. This session will review key federal legislative regulations that case managers need to be aware of.

Additional Learning: Compliance with healthcare legislation is critical for case managers as well as other healthcare professionals. Session 10: Reimbursement Systems
Reimbursement issues are a challenge for all case managers in ensuring patients have access to care. Session Ten will review the major reimbursement systems so case managers have a working knowledge of each.
Session 11:

Legal and Ethical Issues Influencing Case Management
Keeping up to date with legal and ethical issues is an important strategy for all case managers. This session will address key terms and provide examples that each case manager can use to relate to legal and ethical issues impacting the practice of case management.

Additional Learning: The 2010 CMSA Case Management Standards of Practice. CMSA's newly revised Standards of Practice for Case Management (revised 2010) is a timely document, which establishes formal written standards of practice from a variety of disciplines. The Standards of Practice for Case Management were first introduced by CMSA in 1995 and then revised in 2002. The Standards provides voluntary practice guidelines for the case management industry. The Standards of Practice are intended to identify and address important foundational knowledge and skills of the case manager within a spectrum of case management practice settings and specialties.

Session 12:

Professional Development
This last session will review important points professionals involved in case management need in order to to stay clinically and professionally competent and to be able to provide the best care for the patients they are called on to manage.

Additional Learning: During this time of upheaval in the healthcare system, the practice of case management is far from standing still. Take a few minutes to listen to the webinar The Future of Case Management Is Now: Investigating New Models and Trends Improving the Delivery of Care Through Coordinated Paradigms, which explores new models, techniques and delivery paradigms that are emerging to counter the changes stemming from healthcare reform and other quality-based trends like value-based purchasing, pay-for-performance and consumer-driven healthcare.

Post-Test and Program Evaluation:

Following each session, there is a 10-question posttest that allows you to gauge what you learned and share information on additional topics you would like us to cover. As the industry continues to change, continuous learning is essential to allow you to stay up to date.

In addition to the posttest, there is a program evaluation to complete. Please let us know what needs to be included in future e-learning programs.

 


Continuing Education Information:

Eleven contact hours will be awarded once the program has been successfully completed. You can take as long as you want to complete the course. There is a lot of information for you to absorb, so you might want to listen to it more than once.

Continuing education credits are available for nurses, certified case managers, disability management specialists, social workers, and behavioral health counselors. Other disciplines will be considered on an individual basis.
