You may also select organizations based on type of plan, state, ZIP code, year established or other criteria. A sample or draft mail piece must be approved by DHI. Shipping & Turnaround

License restrictions. A signed license agreement prohibiting republishing the data in any form is required on all orders. Data may be used for an unlimited number of mailings within one year of the license date. Data may not be used by parties other than licensee without DHI's written permission. Prices and counts subject to change.

Orders ship within five business days except in November and December, when 7-10 business days may be required. Orders are shipped via UPS Ground at a cost of $9.00 unless another method is requested.

Payment terms. Prepayment is required on initial orders with 30-day net terms for repeat customers. Orders are non-returnable once processed.

Shipping & turnaround. Orders are shipped within five days of receipt of payment and a signed license agreement, and are non-returnable once shipped. Orders are shipped UPS ground at cost of $9.00. Two-day air service is available for $15.00.
