Case Managers: Vital Pieces Within Accountable Care Organizations

Providers, health plans, governments and municipalities, employers, and consumers are looking for innovative approaches to address challenges associated with quality, efficiency, care delivery and coordination, and cost-effectiveness.
 
The Affordable Care Act included potential solutions, but one in particular – accountable care organizations (ACOs) – is positioned to make the biggest impact. ACOs are designed to realign incentives within the delivery system to better meet these challenges and improve outcomes. Accountable care capabilities include:
  • Leadership & Governance.
  • Strategy.
  • Incentive Alignment.
  • Information Technology.
  • Business Operations.
  • Care Management.
Another change that has come out of the ACA is the process of incentivizing care teams to achieve improved clinical outcomes through coordinated, efficient care delivery – effectively shifting incentives from “volume” of care provided to “value” of care provided, which is a significant conceptual shift for traditional delivery models. This shift will change the way care is delivered and paid for across the healthcare continuum.
 
As organizations implement changes, case managers can be leaders in this area. Today, innovative organizations across the country led by case management professionals are implementing new policies, educating professionals into new processes and seeing significant results.
 
On August 30, 2012, as part of our Medical Home Webinar Series, we take a look at innovative programs case managers are leading that are changing the delivery of care, closing gaps in treatment and engaging consumers to take a more active role in their health and healthcare that is improving care and containing escalating healthcare costs.
