Why are you offering the Care Coordination Summit for professionals working along the points of transition of care?
Health reform and other trends affecting medical management are placing a heavy emphasis on securing transitions of care to cut back on recidivist tendencies like unnecessary readmissions and poor medication utilization. The overarching reason for the Care Coordination Summit operates on the belief that health care teams must unite to adequately address the emerging trends of 21st century care.
Do you offer group rates?
Yes. The Care Coordination Summit on-demand program allows for the interdisciplinary to come together for a full day of learning. We do offer reduced rate for groups. Please contact Hope Kabik for applicable discounts.

Will breakfast and lunch be served?
Yes, there will be a continental breakfast, a luncheon and several breaks.
Can I download/view materials in advance of the event? Yes. We will make the speaker slides available through our continuing education portal prior to the event. Detail will be sent in an email about one week prior to the event.
How to I get me CEUs? At the conclusion of the event you will be able to log into continuing education portal and take our course evaluation. Upon completion of this, you'll be able to print our your certificate of attendance.
