This year, the week of October 9-15 is designated as National Case Management Week. This year’s theme is Case Management, the GPS for Quality Healthcare. This is a time for all case managers to share their successes and celebrate the important work they do as part of the healthcare team.
Dorland Health is doing its part to recognize and support the important work case managers across the care continuum are doing to improve the patient experience, improve quality and contain escalating healthcare costs. Our mission is to provide information, recognition, tools and resources you and your team can use to meet the needs of the patients you care for. Here are a few of our upcoming activities that I hope you will take advantage of:
- Dorland People Awards: The Dorland People Awards recognizes professionals who bring creativity, innovation and mentoring skills to the practice. The finalists were announced weeks ago and we are in the final stages of preparation for our Awards Luncheon that will take place on October 20, 2011, at the National Press Club. The October/November issue of Case In Point will highlight the winners and the honorable mentions. There is still space for you to attend the celebration.
- Care Coordination Summit: Advancing the Delivery of Care: Our 3rd Annual Care Coordination Summit will address the cutting-edge trends and issues impacting the delivery of care regardless of setting. The Summit will take place on October 21, 2011, at the National Press Club. Join your peers and our esteemed faculty as they present issues impacting the delivery of care and share best practices that all who attend can take back to their organizations to improve the practice and outcomes. - Case In Point Webinar Series: Each month, we feature webinars that address topics that are on the top of the radar screen for all members of the care coordination team. If you have not listened live, our programs are archived and available on demand for you and your team access as part of your professional development plan. Our October 26th webinar is titled The Future of Case Management Is Now: Investigating New Models and Trends; Improving the Delivery of Care Through Coordinated Paradigms. The program brings together three experts to discuss new models, techniques and delivery paradigms that are emerging to counter the changes stemming from healthcare reform and other quality-based trends like value-based purchasing, pay-for-performance and consumer-driven healthcare.
- This webinar is a perfect way to ensure that the excitement and attention your case management team receives from celebrating case management week carries through for the rest. The program will support your efforts to ensure every case managers understand their role, function and responsibilities in ensuring each patient receives safe, effective, evidence-based care across the care continuum.
- Medical Home Webinars: A new project that we are working on is developing a series of webinar to provide information professionals involved in the Patient Centered Medical Home can use as they move forward with this new model of care. Case managers figure prominently in this new model so it is with great excitement we rolled out the Webinar on September 27, 2011. The title was; How To Implement a Medical Home Strategies for Providers, Payers and Involved Practitioners the program is available on demand for you and your entire team. - Our next webinar will focus on a topic that is very important for professionals working with seniors. Tamperproof: Securing Medication Reconciliation What You Need to Know to Improve Adherence and Enhance Patient Safety The program will be held on November 15, 2011 so make sure you review the program and register your team today.
